Hospital price-transparency enforcement

Since January 2021 federal rules (45 CFR part 180) require every hospital to post its standard charges online — a machine-readable file of all items and services and a consumer-friendly list of shoppable services. CMS enforces the rule in steps, and publishes every step. Its list holds 13,355 actions: 3,646 warning notices, 2,107 requests for a corrective action plan and 29 civil monetary penalty notices.

How enforcement works

  1. Warning notice. After a complaint or its own review, CMS tells a hospital what is missing from its posted charges.
  2. Corrective action plan. If the problems are not fixed, CMS requests a plan with a deadline.
  3. Civil monetary penalty. A hospital that does not submit or follow its plan can be fined; penalties scale with bed count (45 CFR 180.90).
  4. Closure. When the hospital comes into compliance CMS closes the case — recorded as "Met Requirements" or a closure notice.

Most cases end at the first or second step. The list shows actions, not outcomes for patients, and a warning is not a finding of wrongdoing.
